Founded in 2014 by three colleagues with extensive clinical, analytical and healthcare management expertise, Fusion48 has delivered a range of successful and well received contracts in integrating health & care services, evaluating service pathways and system resilience, general practice development, and training and education in frailty and older people's services.

Notable milestones include the award of a Health Education England multi-year contract to develop and deliver the Frailty & Supported Self-Care Toolkit and Training for Nottinghamshire in June 2015. Whilst in September 2016, Fusion48 were awarded a grant through the Ufi charitable trust to develop their highly innovative and successful FrailtySIM Virtual Reality experience and associated training modules for the care sector. 

In May 2018, Fusion48 was the first organisation to analyse the impact of the first year of the 2017/18 GP contract requirements related to routine frailty identification and frailty care.    

When the full extent of the COVID-19 pandemic hit the UK in March 2020, Fusion48's Frailty360 training had already been delivered to thousands of people across the NHS, social care, voluntary and other related sectors.  Over 300 'Frailty Champions' had also been trained using the person-centred approaches and they continue to cascade the training across health and social care systems the length and breadth of the United Kingdom.

Whilst Fusion48 is no longer able to offer face to face training events, the Frailty360 resources are available at https://frailty360.org/  and through the Frailty Toolkit. 

David, Sunil and Dawn have all continued to broaden their clinical and health data portfolios and their geographical reach. 

Whilst we now have reduced capacity to directly assist organisations in the delivery of effective and efficient care, we remain open to opportunities to collaborate so please do get in touch.
