Extra session added to Every Colleague Matters programme

A third Raising awareness of frailty and supported self-care session has been added to the Every Colleague Matters programme on 10th February 2016. The morning session is now fully booked and there are limited places available for the lunchtime slot. The extra session takes place from 1430 - 1630. Read more

The Frailty Fulcrum featured on NHS England website

The concept of frailty as a long-term condition brings with it the opportunity to adopt a much more proactive, person-centred, community-based approach to care. Achieving this requires a new approach to care, particularly through supporting self-care. This new approach will be enabled through a wider awareness of frailty and a greater shared understanding of the condition. The ‘frailty fulcrum’ is an animated model for frailty that has been developed with these opportunities in mind and features in a guest blog by Dr Dawn Moody on NHS England website. Read more

Bookings open for Every Colleague Matters week in Nottingham 8 - 12 Feb 2016

We are delighted to be a part of Nottingham City Council's Every Colleague Matters event. This series of Awareness Raising and Skills Development sessions are open to everyone who works within the Children's and Vulnerable Adults Workforce in the City of Nottingham. The event includes the "Raising awareness of frailty and supported self-care" sessions on 10th February that will be led by Dr Dawn Moody. Read more

Fusion48 involved in two of the projects to feature at the King’s Fund Frailty event.

The King's Fund has announced the full programme for the Delivering integrated care for older people with frailty conference on 16th March 2016. Fusion48 is delighted to be involved with two of the chosen projects which have "demonstrated innovative approaches to delivering integrated care for older people with frailty that will provide delegates with ideas and inspiration". Assessing the impact of GP Practice Development Workshops in Norfolk & Waveney

Between mid January and early March 2015, Norfolk Local Medical Committee commissioned Fusion48 to provide a series of five CQC Inspection Preparedness workshops for practices across the area. To understand whether the training had any long term benefit to the participating practices, we have just concluded a 6+ month follow up survey to find out how practices have been getting on and what they valued most about the workshops and supporting materials. The Nottinghamshire Frailty Toolkit & Training - November 2015 update

The first wave of training events and the launch of the Frailty Toolkit took place in October 2015. To date, over 150 health and care professionals representing more than 20 organisations from across the NHS, Adult Social Care, Private and Third sector providers have joined the programme. Read more
