Joining up care for older people with frailty

A new joint report from the Royal College of GPs and the British Geriatrics Society has been published today, showcasing how GPs and geriatricians are collaborating to design and lead innovative schemes to improve the provision of integrated care for older people with frailty. We're delighted that amongst the 13 case studies is an example from North East Yorkshire that makes reference to the Frailty Fulcrum and Nottinghamshire Frailty Toolkit. Read more

Extra session added to Every Colleague Matters programme

A third Raising awareness of frailty and supported self-care session has been added to the Every Colleague Matters programme on 10th February 2016. The morning session is now fully booked and there are limited places available for the lunchtime slot. The extra session takes place from 1430 - 1630. Read more

Assessing the impact of GP Practice Development Workshops in Norfolk & Waveney

Between mid January and early March 2015, Norfolk Local Medical Committee commissioned Fusion48 to provide a series of five CQC Inspection Preparedness workshops for practices across the area. To understand whether the training had any long term benefit to the participating practices, we have just concluded a 6+ month follow up survey to find out how practices have been getting on and what they valued most about the workshops and supporting materials. Simple but effective: Innovative older people’s project report and resources published

The report and resources from an innovative older people's project run by GP practices across the Newcastle-under-Lyme South Locality in North Staffordshire has been published. The key intervention was a home visit by an Elderly Care Facilitator to complete a very comprehensive and holistic older person's assessment. This resulted in a follow-up action plan involving organisations and services across health, social care, local charities, police, fire service and local businesses. The project has been extremely well-received by older people, their carers and staff alike... Read more
