  1. Identify frailty in primary care using a validated tool
  2. Take opportunities for frailty identification
  3. Know when to carry out a comprehensive geriatric assessment (CGA)
  4. Conduct a CGA in primary care
  5. Recognise the important role of carers
  6. Ensure integrated care pathways are in place
  7. Carry out a comprehensive medication review
  8. Identify and manage co-morbidities that affect mental wellbeing
