Supporting Integration in Older People’s Services

“Helping to put together the pieces of an older people's service”
Fusion48 have been working with a CCG in the North West to develop the building blocks for the delivery of an integrated older peoples service. We have carried out reviews of
and have given ‘critical friend’ support to the development of the System Resilience Plan.
Each clinical service project was approached using our characteristic combination of:
- Data analysis (of both contextual and service specific data)
- Stakeholder interviews
- Audit of clinical activity
- Case study presentations
- Review of frailty themes
We worked with a wide range of stakeholders including CCGs, social care commissioners and providers, acute and community service providers, ambulance service, third sector organisations, patient representatives and primary care teams.
Project findings were presented back through a range of formal presentations, round table discussions and workshop events. Feedback from stakeholders was excellent and the findings are now being used to inform the integration and older peoples service development agenda in this health and care economy.
